If you are disabled, you must get in touch with NDIA and ask for (Access Request Form) ARF. The NDIS has simplified the application process for disabled persons in order to assist them. The ARF can be downloaded from https://www.ndis.gov.au/how-apply-ndis/what-access-request-form#access-request-form . If you are unable to go online,in order obtain an access request form, call 1800 800 110. Alternatively, you can download the ARF save it to your computer, fill it out, and email it to the NDIA along with the supporting documentation.

Once you have completed the ARF you need supporting documentation. You can have your treating health care professional to fill out Supporting Evidence Form (SEF). The National Disability Insurance Agency (NDIA) needs this form to determine whether you qualify for the NDIS.

The NDIA needs evidence for your age, residency, and disability. Additionally, you can be requested for some more details concerning your condition. It’s crucial to describe how your condition impacts your capacity to carry out daily activities.

Once you have completed ARF, SEF and has proof of you age, you can send all of this information to

  • email to NAT@ndis.gov.au, or
  • post to GPO Box 700, Canberra, ACT 2601
